• Partnership Deed / MoA / MoU / YV / LLP
• letter of authorization
• Photograph of signboard of establishment/shops
• Signature with the designation of the employer
• Signature of Applicant
• Occupation certificate from legal requirement such as ISEBI / IRDA / RBI / Home Special Department
